Double-pronged attack could treat common children's cancer

A dual-pronged strategy using two experimental cancer drugs together could successfully treat a childhood cancer by inhibiting tumour growth and blocking off the escape routes it uses to become resistant to treatment, finds a new study.


Scientists at The Institute of Cancer Research, London, found that combining two separate molecularly targeted therapies could stop processes driving growth in a cancer called rhabdomyosarcoma, a major cause of cancer death in children.


The drugs, called AZD8055 and AZD6244, block two different signalling pathways involved in cancer growth acting like road-blocks on two separate routes that cancers could otherwise use to evade treatment.


The study, published in Clinical Cancer Research today (Friday, 1 November), was funded by the NIHR Biomedical Research Centre for Cancer at The Royal Marsden NHS Foundation Trust and The Institute of Cancer Research (ICR), with additional funding from Cancer Research UK, The Royal Marsden Hospital Charitable Fund and the Chris Lucas Trust.


Rhabdomyosarcoma tumours can form anywhere in the body and resemble primitive muscle tissue. Despite advances in treatment options, there has been little improvement in outcome for patients with rhabdomyosarcoma in decades and they remain difficult to treat.


Previous research has shown that many rhabdomyosarcomas display activity of the PI3 Kinase signalling pathway, which plays a key role in cancer growth. However, blocking this pathway in other cancer types can lead to alternative signalling pathways becoming active to compensate, allowing resistance to treatment to develop.


In this study, scientists at the ICR targeted the PI3 Kinase pathway and a second pathway called MAP Kinase, to assess any compensatory signalling and determine if blocking both pathways could effectively inhibit rhabdomyosarcoma cell growth.


The researchers found that the PI3 Kinase pathway was active in 83% of rhabdomyosarcoma samples from patients, and that 43% of these also showed activation of the MAP Kinase pathway. In experiments on rhabdomyosarcoma cells to block either pathway alone, they saw compensatory signalling through the alternative pathway, suggesting that inhibiting both pathways is an essential approach to treatment, irrespective of whether MAP kinase signalling was initially activated.


The researchers tested rhabdomyosarcomas with drugs known to be effective against the PI3 Kinase and MAP Kinase pathways. When they tried the drugs AZD8055 and AZD6244 separately they saw reduced cell growth and a decrease in levels of markers showing the activity of the signalling pathways. However, compensatory activity was clearly evident.


But when they combined the two drugs they found a synergistic effect, with cell growth reduced to a greater extent than with either treatment alone. They saw similar synergistic results when AZD8055 and AZD6244 were used together in mice with rhabdomyosarcoma tumours, with tumour marker levels reduced to less than 30% of those in controls.


Co-author Dr Janet Shipley, Team Leader in Sarcoma Molecular Pathology at The Institute of Cancer Research, said:

"Rhabdomyosarcoma is the main type of sarcoma to affect children and little improvement has been made recently using conventional treatments like chemotherapy and radiotherapy - survival rates for some patients with this disease remain bleak. More effective targeted treatment is desperately needed. Our study shows that treating with one or other of these two drugs is not a good strategy but that combining them is a very promising option."


Co-author Dr Jane Renshaw, Senior Scientific Officer at The Institute of Cancer Research, said:

"We found that while most rhabdomyosarcoma tumours seem to have active PI3K signalling, inhibiting this pathway alone isn't enough to be an effective treatment. Cross-talk between the PI3 Kinase and MAP Kinase pathways means that cancer is able to find an alternative route, like traffic finding a way around a road-block. Targeting both pathways using two drugs together stops that compensatory action.


"These two drugs are being tested for use against cancers in adults so the next step will be to progress with clinical trials for children using the dual approach."


Nell Barrie, Cancer Research UK's Senior Science Communication Manager, said:

"Understanding the inner workings of cancer cells is crucial to finding the best ways to tackle the disease. This lab research emphasises the importance of targeting each cancer's weak points and combining drugs to develop more effective treatments which are urgently needed to improve survival for children's cancers like rhabdomyosarcoma. Further research and clinical trials will shed light on whether this promising drug combination could help save more lives."

AP source: Martin out with emotional issues

DAVIE, Fla. (AP) — Miami Dolphins tackle Jonathan Martin left the team this week to receive professional assistance for emotional issues, a person familiar with the situation said, and was ruled out of Thursday night's game against Cincinnati.

Martin was with relatives, and his issues didn't involve any problems with the coaching staff, the person said. The person spoke to The Associated Press on condition of anonymity because the Dolphins have said not released any details of the illness.

Martin, 24, played in Sunday's loss at New England, then missed practice this week. He was replaced by Tyson Clabo, who started Miami's first six games before being benched.

A second-round draft pick from Stanford, Martin started every game at right tackle as a rookie last year. He switched to left tackle this season, then moved back to the right side last week in a reshuffling of the Dolphins' struggling offensive line.

"You can approach this two different ways," Martin said last week regarding his latest position switch. "You can go in the tank and be one of those guys that moans and is a cancer in the locker room, or you can be a professional and play as hard as you can. My mindset is I'm going to go out there and do whatever I can to help the team win."

Pass protection has been a problem for Miami all season. Ryan Tannehill went into the Bengals game with an NFL-high 32 sacks, and the Dolphins (3-4) were saddled with a four-game losing streak.

Martin's agent didn't respond to requests for comment.

In an unrelated move, the Dolphins promoted receiver Ryan Spadola from the practice squad to the active roster.

Inside The Dungeon of Doom: Kevin Sullivan on wrestling's wackiest group










In summer 1995, one year before the emergence of The nWo, WCW was stuck in a seemingly inescapable limbo. The era of Ric Flair’s thrilling rivalries against Vader, Sting and Ricky Steamboat was in the rearview mirror. Hulk Hogan had arrived one year prior, but he wasn’t being accepted by Atlanta crowds with the same maniacal frenzy that had stirred up WWE fans in the ’80s.

The lead producer of WCW at the time, Kevin Sullivan – a Boston-bred veteran brawler – needed to come up with something. He needed to do it fast. And what he came up with might be the single most absurd narrative that has ever unfolded in one of the major sports-entertainment organizations — The Dungeon of Doom.

A cadre of cartoonish villains that assembled in a haunted fortress, The Dungeon grew and grew to amass no fewer than 20 individual members, each more ridiculous than the next. Watching the group’s television segments today is a surreal experience and plays like a B-movie out of the mind of Troma’s Lloyd Kauffman. There were bizarre sci-fi elements like teleportation, Hogan’s turn to “the dark side” long before going Hollywood and even the first on-screen appearance of Big Show.

With the rise of YouTube, the group's run has developed a cult following thanks to its cheap production values and endlessly quotable lines like, “It’s not hot!” Fascinated by the the macabre world of The Dungeon of Doom, and the notion that its existence overlapped with the intense realism of The nWo, WWEClassics.com set out to discover the inside story. We sat down with Kevin Sullivan, the Dungeon's Taskmaster, to find out what made the group tick and why it even happened at all.

There is a corny saying that college football coaches like to use that goes something like this: "They remember what you did in November."

The first weekend of November features a light slate of big games, another marquee matchup in Atlantic Coast Conference that could get ugly and a Big Ten rivalry that has been turned upside down in recent years.

Five things to know about in Week 10 of the college football season.

SUNSHINE STATE BEATDOWN? Miami-Florida State games bring back a rush of great college football memories. Wide Right. Wide Left. Games filled with future NFL players and Heisman Trophy winners. This latest matchup of the Hurricanes and Seminoles will be the first since 2003 in which both teams are ranked in the top 10 deep into the season. But while it's clearly the most interesting game of the weekend in terms of rankings (Florida State is No. 3 and Miami is No. 7) and brand names, that 21½-point spread hints that another Seminoles' blowout similar to the 51-14 beating it put on Clemson is very possible in a matchup of a good team (Miami) versus an elite team (Florida State).

LITTLE BROTHER: In 2007, after Michigan beat Michigan State for the sixth straight time, Wolverines star tailback Mike Hart referred to the Spartans as the little brother of the rivalry. Well, the little brother has grown up and has been taking it to big bro lately. The Spartans ripped off four straight victories after that 2007 game, matching Michigan State's longest winning streak in a long-running series that has mostly been dominated by Michigan (68-32-5). Michigan snapped that streak last year with a 12-10 victory. Expect something similar this season. Michigan State's defense is one of the best in the nation and both teams have offensive issues.

RESTING AND GETTING READY: There are only 14 games, including Thursday night's matchup between No. 25 Arizona State at Washington State, involving ranked teams this week, the fewest of any week so far this season. Why? Well, the calendar broke in a way that put an extra week in the college football season this year and it's allowed some teams to have as many as three open dates. Plenty of top teams are taking advantage of that by getting a little extra rest before huge games next week. On Thursday, Nov. 7, No. 2 Oregon is at No. 6 Stanford and No. 5 Baylor hosts No. 13 Oklahoma. Then on Nov. 9, No. 1 Alabama faces No. 11 LSU.

LAS VEGAS BOWLING: Among the 17 FBS teams that can reach six wins this week is UNLV (5-3). The Rebels host Mountain West Conference rival San Jose State (4-3). A victory makes UNLV bowl-eligible. The Rebels haven't played in a postseason game since the 2000 Las Vegas Bowl.

ARMY GROUNDING AIR FORCE: Since 1989, Army has beaten Air Force just three times, including last season's 41-21 victory at West point, N.Y. The Cadets (3-5) are trying to make it two straight against the struggling Falcons (1-7) in Colorado on Saturday. Army's last winning streak against Air Force came when it won three straight from 1976-78. The Falcons, already assured of their first consecutive losing seasons under seventh-year coach Troy Calhoun, haven't started a season this poorly since going 0-8 in 1979. Their first victory that season came against Army.

For The Tablet Generation, A Lesson In Digital Citizenship

Parents pack into a gym at Cahuilla Desert Academy, a middle school in the southern California city of Thermal. The near triple-digit daytime heat of the Coachella Valley, southeast of Palm Springs, has given way to a cool evening. It's iPad information night.


Before addressing the crowd, Principal Encarnacion Becerra talks up the district's ambitious new iPads-for-all initiative with the fervor of a Silicon Valley entrepreneur.


"It's truly a revolution, what's happening," he says. "Technology has finally caught up to where truly you hold the Internet in the palm of your hands. The power of the mobile devices that exist now — we have to have to leverage that capacity and to evolve as educators to address those needs."


Coachella Valley Unified — a predominantly low income, rural and Latino school district — is in the process of handing out iPads to every student, pre-kindergarten through 12th grade. Kids seventh grade and up get to take the device home evenings, weekends and breaks. Voters approved a bond issue to pay for it.


Administrators here paint it as a modern civil rights issue. Technology tools, they argue, will help boost achievement, prepare kids for today's workplace and narrow the digital divide between poor and wealthy areas.


A growing number of school districts across the U.S. are handing out tablet computers and integrating the devices into their curriculum. But the old issue of how much Web access kids should have on school-issued devices is growing more complicated as kids surf on multiple devices and access multiple networks at home, school, public hot spots and more.


iPad Security


Last month students at the Los Angeles Unified School District easily got around a security firewall on their district-issued iPads and could surf wherever they wanted. LA has now slowed down its iPad rollout amid growing concerns about LA's entire tablet project.


This worries Joey Acuna Jr., father of a student in Coachella Valley Unified.



"I have concerns after hearing what happened in LA Unified," Acuna says. "Kids are kids, and they're going to try to do what they think they can get away with. And not to be mean, but sadly ... some of our kids probably have better knowledge of these kind of electronic devices than some of our teachers."


LA is now exploring new security tools to block access to certain sites, including social media sites and YouTube. "All social media sites are blocked," says LA school district spokesman Thomas Waldman.


Parents here in Coachella want to know whether their district has learned from LA's missteps.


The Coachella Valley school district will block certain sites deemed harmful and install a tracking mechanism and other tools to monitor kids' use. Part of that falls under the Children's Internet Protection Act: Schools and libraries that accept certain federal funding for technology must install Web filters to shield kids from pornography and explicit content online.


But the district is taking a more nuanced approach than LA Unified to the access and use of social media sites. They're not blocked. The idea now is to educate kids and parents about appropriate use of the iPad — or what the district calls online ethics and digital citizenship.


Karen Cator, CEO of the nonprofit education group Digital Promise, says the issue of filtering is incredibly complicated because the Internet is continuously changing.


"I think it's futile to try to shut this down completely," she says. "And it's a missed opportunity, if we do that, to teach kids how to act appropriately in what will be their lifelong globally networked world."


Setting Up Rules


Eighth grade physical science teacher Tim Sharpe at Cahuilla Desert Academy has been using the iPad in a pilot program for more than a year. He says tablets are tailor-made for science learning: His students use them to take photos, write about labs and tap into the latest educational science apps.


Sharpe has already confronted the problem of renegade surfing on mobile phones. Students can get on YouTube with their smartphones, he says, but they know Sharpe might take their phone away for the day if they do.


What sites to block, beyond the ones legally required, should be a teacher-student classroom management issue, he says.


Sharpe devised a system that engages kids and rewards them: If they finish their iPad project on time, they can then play games or take pictures for fun with the devices.


"And there's ... a point system," he says. "So you just lay the rules down. And I find that the kids go with that."
